However, the French side mounted an incredible comeback with goals from Corentin Tolisso and Alexandre Lacazette to force the match to extra-time, though Tolisso was sent off just seconds before the whistle for a second yellow.
Remarkably, the one-man deficit seemed to spur Lyon on as they came flying out of the traps in extra-time with Rayan Cherki netting a brilliant strike.
Matters went from bad to worse as Luke Shaw conceded a penalty which Lacazette converted, prompting fans to leave Old Trafford with United two goals down in extra-time.
But it was time for United's comeback, with Casemiro being fouled in the box moments later which led to a lengthy VAR check before Fernandes converted from the spot.
Mainoo's equaliser and Maguire's winner with minutes of each other capped off a special European night for the Red Devils, who are desperate to win the competition.
